Twelve Oaks Treatment Staff
All of our staff members are highly qualified professionals in the field of addiction and alcoholism, and are skilled in a variety of treatment modalities. They are nurturing individuals chosen for sensitivity and insight. These caring people, many of whom have been through recovery themselves support and work together in the therapeutic environment toward a common goal: to help those suffering from the diseases of addiction and alcoholism. This is a setting in which clients receive the tools they need to begin a successful recovery process.
As a result of the quality of the services provided at Twelve Oaks and the expertise of its medical and clinical staff we have been chosen to be an Approved Clinical Preceptor Affiliate Training Site (teaching facility for medical students) for the Lake Erie College of Osteopathic Medicine.
STAFF MEMBERS:
Keith Kuhn, MBA, MSW, CADC - Executive Director
Keith is an established health professional with extensive leadership experience. Most recently, he served as Vice President, Adult Services for a substance abuse and mental health services corporation in Illinois where he managed clinical and administrative services for three facilities. There, he developed and implemented a new co-occurring disorders program that included specialized programming for Veterans, and achieved an outstanding 98% average occupancy in one of his assigned residential treatment units. Prior to this, Kuhn was Executive Director for another substance abuse and mental health services company for four years where he improved client satisfaction scores by nearly 10 percentage points and worked with federal government agencies and private insurance companies to obtain new coverage contracts.
Keith, a Certified Alcohol and Drug Counselor (CADC), also has been Director of Outpatient Programs for a behavioral health services corporation where he managed psychiatric and counseling programs and clinics for over 10,000 adults, children and families. He worked as a chemical dependency counselor and behavioral health therapist prior to his executive positions.
Keith earned his BA and Master of Social Work from Arizona State University and also has an MBA in Healthcare Management. In 2004, he was appointed an associate/adjunct faculty member for Arizona State University and for University of Illinois – Springfield, where he taught in the Schools of Social Work and Human Services, respectively.
B. Diane Vchulek, MS, MA, LMHC, BCETS PTSD – PTSD Coordinator, Program Director
Diane specializes in helping people heal from stress reactions and responses associated with both civilian and military traumatic experiences. She has developed ASPIRE, a tract specifically for Twelve Oaks' patients for the treatment of addiction and PTSD for military and civilians.
Diane has presented workshops nationally and internationally for the International Society for Traumatic Stress Studies (ISTSS) and the International society for the Study of Trauma and Dissociation (ISSTD)
Diane has broad experience as a Licensed Mental Health Counselor spanning over 25 years. Diane was in private practice for 15 years treating individuals with PTSD and other issues related to trauma reactions and addiction. Diane also has clinical experience in inpatient facilities, residential treatment, and specialized programs for trauma related difficulties.
Diane's experiences with the military throughout her life provide insight into her work with military veterans suffering from combat related traumatic stress, as well as with the family members who support them.
Diane is also an Adjunct Professor in Psychology and is also currently completing her dissertation for Ph.D. requirements focusing on “Stress Reactions in Military Wives of Deployed Spouses in Desert Storm/Shield and Operation Iraqi Freedom/Enduring Freedom”

Dr. Reeves is a graduate of Florida Southern College with a Bachelor’s of Science Degree with Honors, where he was also awarded the Outstanding Pre-Medical Student Award. He then attended The University Of South Florida College Of Medicine in Tampa, Florida, and graduated with a Doctor of Medicine degree. Dr. Reeves completed his five year General Surgery internship and residency at Carolinas Medical Center in Charlotte, North Carolina, and after serving as a General Surgeon in Okinawa and Pensacola for the US Navy, He performed a two year fellowship in Vascular Surgery at the University of Tennessee Medical Center. He then continued to serve in the Navy as a Vascular Surgeon at Naval Medical Center San Diego, and aboard the USNS Mercy, and the USS Constitution. After another fellowship at Stanford University, he was selected to Head the Division of Vascular Surgery in San Diego.
Upon completion of his Navy service Dr. Reeves returned to his hometown of Fort Walton Beach to private practice, where, while practicing Vascular Surgery, a huge unmet need presented itself, and he began treating the disease of addiction, including chronic pain with addiction. Dr. Reeves then obtained his third Board Certification, becoming certified in Addiction Medicine at which time he began to focus his practice exclusively on this disease. As his practice expanded he became the Medical Director of Access Recovery Solutions, an outpatient treatment center in Panama City, and also the Medical Director of a 102 bed residential treatment facility, Twelve Oaks, in Navarre, Florida. He still maintains his private practice in Family Practice and Addiction Medicine at South Walton Medical Center in Miramar Beach, Florida.
Chris Campbell, MS, - Lead Counselor
Chris is certified by the State of Florida as a Mental Health Professional. Chris completed his Master of Science degree in Psychology from Troy University with a primary area of focus in mental health. Chris originally came to Twelve Oaks as an intern in 2004 and has been the Program Director since mid 2009. Chris has worked in the field of counseling/mental health/substance abuse for many years and is no stranger to the field of counseling.
Chris spent 30 years in the military and had the opportunity to experience various people, places, and things which enable him to easily understand the various backgrounds and culture of our clientele. Often our patients are dealing with personal conflicts and problems which Chris has experienced with others during his military career. Chris is very capable of helping patients work through their problems as they journey down the road of sobriety. Chris’ military experience also enriches us in our ability to deal with military personnel whether they are here for treatment or referring a patient to us.

Maria Rogado, MS – Adult Therapist
Maria has a Masters in Psychology and Counseling from Troy University with a background in substance dependence treatment. Maria is dedicated to the client and uses a client-centered approach to counseling. Maria demonstrates a sympathetic, nurturing, non-judgmental attitude and provides a comfortable environment to initiate change.

Kristin DeCorte, MA, LMHC - Adult Therapist
Kristin graduated from the University of Southern Mississippi with a Bachelors of Science in Psychology (1996), and earned her Masters of Arts in Counseling Psychology (2006) from Southern Christian University (now Amridge University) in Montgomery, Alabama.
A Licensed Mental Health Counselor, Kristin has experience working with a diverse population spanning children, adolescents and adults. Focused on treating mental health and substance abuse/dependence concerns utilizing a non-judgmental, client-centered behavioral approach to change, Kristin is dedicated to her work and the clients served.

Don Harrison, M.A., LMHC - Adult Counselor
Don has a Master of Arts degree in Counseling Psychology from the University of West Florida with a background in Mental Health and Dual Diagnosis (co-occurring Substance and Mental Health problems). Mr. Harrison is a Licensed Mental Health Counselor in the State of Florida and has worked as a counselor with a non-profit charity as well as in an Acute Psychiatric Hospital.
Don's focus with patients is holistic, recognizing that recovery from addiction entails encouragement of physical, mental, emotional and spiritual balance and growth in each patient’s recovery program. His approach is aligned well with Twelve Oak’s dedication to recovery, renewal, and rebuilding of life.

Traci Poston, MS, LMHC, CAP - Adult Therapist
Traci is a Licensed Mental Health Counselor and Certified Addictions Professional in the state of Florida. Traci graduated from the University of Illinois at Springfield (formerly Sangamon State University) with her BA in Psychology in 1990 and her MA from Troy State University in Counseling and Human Development Counseling with a focus on Mental Health Counseling in 1995.
Traci has over 20 years of treatment experience working with adolescents and adults. She has worked in outpatient, residential, and correctional settings, as well as in a youth runaway shelter. She has worked as an independent contractor completing comprehensive behavioral assessments on infants and adolescents whose families are involved with the Department of Children and Families. She has also provided therapy for individuals mandated to complete a Batterer’s Intervention Program. Traci is also a Qualified Supervisor with the State of Florida to provide supervision to clinicians seeking licensure as a mental health counselor.
Traci’s experiences have allowed her to develop insight into the correlation between chemical dependency as well as mental health issues such as depression and anxiety. She is also empathetic and compassionate towards individuals who suffer from chronic pain conditions whereby substance abuse and/or mental health problems have played a role. Traci enjoys educating people on how to manage their issues utilizing a holistic approach to recovery and overall wellness.

Yvonne has a Masters in Social Work from Florida State University. Yvonne has worked in the field of substance abuse and chemical dependency since 2006. She began her career at Twelve Oaks as a BSW intern from the University of West Florida. Upon completion of her internship, Yvonne accepted the position of case manager at Twelve Oaks. She moved on in 2007 to pursue various aspects of the field before returning to Twelve Oaks in 2011 as an adult and adolescent counselor, and the adolescent program coordinator. During her time away from Twelve Oaks, Yvonne has worked with adult and adolescent patients in inpatient and outpatient settings, as well as, within the prison system.
Yvonne believes in having clear expectations and boundaries in her program. She is firm but fair. Her law enforcement background in the U.S. Air Force is beneficial to the facility, the program, and the patients

Nicole YL Ward, MS, CMHP – Adult and Adolescent Therapist
Nicole graduated from the University of Nebraska in 2002 with a Bachelor of Science in Family Science and received her Masters of Science in Human Relations through the University of Oklahoma in 2005.
Nicole has worked in the field of substance abuse and chemical dependency since 1999. Nicole has experience working with adolescents as well as adults in outpatient and inpatient settings. Nicole has also worked closely with probation and other legal services to include drug courts, probation/community control officers and attorneys to help clients achieve their success in recovery. Nicole was a counselor in the United Kingdom helping adolescents become more educated about substance use and abuse as well as helping them to overcome their addictions.
Nicole grew up in a military family and is presently a military wife and understands the struggle that substance use and abuse plays in military and civilian families. She has also worked closely with the United States Air Force and the Office of Special Investigations (OSI) as the Drug Testing Program Manager.
