All of our staff members are highly qualified professionals in the field of addiction and alcoholism, and are skilled in a variety of treatment modalities. They are nurturing individuals chosen for sensitivity and insight. These caring people, many of whom have been through recovery themselves support and work together in the therapeutic environment toward a common goal: to help those suffering from the diseases of addiction and alcoholism. Mr. Lehmann has a Masters Degree in Human Services Administration and over 30 years of progressive management experience in the substance abuse and psychiatric treatment industries. He has been in senior leadership positions managing psychiatric hospitals, chemical dependency treatment facilities, adolescent residential treatment programs and community mental health centers. He has also held positions as a Vice President of Operations managing multiple chemical dependency treatment facilities across the country.William F. Murphy, D. O.
Program Medical Director Board Certified in Addiction Medicine – Diplomate, American Society of Addiction Medicine Board Certified in Family Practice, American Osteopathic Board of Family Medicine Dr. Murphy is an experienced physician who has been in practice for 30 years. He was among the first group of physicians in America to be recognized as “board certified” in addiction medicine. Most importantly, Dr. Murphy listens to the patient and cares about those things that are important to the patient.
Other honors include: A Vietnam combat infantry veteran – received a Purple Heart and Combat Infantry Badge Twice past president of Florida Osteopathic medical Association, District 7. A recipient of the Wendall Rollason Award, being recognized as the “model physician” for the healthcare systems in the State of Florida. A recipient as a Fellow in the American college of Occupational and Environmental Medicine.
Franklin Beall, M.S., Certified Mental Health Professional
Graduate of Troy University with a Masters in Psychology in Counseling and a certified mental health professional with a background in community mental health and substance abuse treatment for the past 6 years. Demonstrating care and compassion for individuals caught in the cycle of substance abuse and committed to providing the skills to empower themselves to make positive change. I am passionate about the field of counseling and it is my privilege to be a part of the lives of those seeking help.
W. Dale Perkins MA, MSW
Dale received a Masters Degree in Clinical Social Work from Florida State University with a 3.97 grade point average. Perkins completed an internship at the Pensacola Veterans Administration Joint Ambulatory Care Center Mental Health Intensive Case Management program working with individuals who suffer from severe and persistent mental illness.
Dale has developed expertise working with patients with a dual diagnosis of mental illness and substance abuse. Additionally, Dale worked for six years as a Master Counselor with emotionally disturbed adolescents in a residential wilderness setting. Loved for his entertaining approach to group work, Mr. Perkins therapeutic recovery groups are among the most popular in Northwest Florida.
Mr. Perkins specializes in motivational interviewing and is a respected innovator in the twelve step facilitation approach. Mr. Perkins also holds a Masters Degree in Political Science from the University of West Florida.
Chris Campbell, MS
is our Lead counselor. He is a Certified Mental Health Professional by the State of Florida. He completed his Master of Science degree in Psychology from Troy University with a primary area of focus in mental health. He came to Twelve Oaks as an intern in 2004. He has worked in the field of counseling/mental health/substance abuse for five years. Chris is no stranger to the field of counseling.
He spent 30 years in the military and had the opportunity to experience various people, places, and things that enable him too easily understand the various backgrounds and culture of our cliental. Often our patients are dealing with personal conflicts and problems that Chris experience with others during his military career. He is very capable of helping them work through their problems as they journey down the road of sobriety. His military experience also enriches the ability for us to deal with our military personal.
He talks their language and understands their struggle as a defender of our country. His therapy style is motivational interviewing, a technique where he becomes a helper in the change process and express acceptance of his client. This allows him to interact with the patients in a way to deal with their decisiveness in helping the patients to realize personal goals. This style of therapy builds on Carl Rogers' optimistic and humanistic theories about people's capabilities for exercising free choice and changing through a process of self-actualization. If you ask him what is so special about his job, he would not hesitate to tell you his life has been so bless an enriched by other until his greatest desire is to give other what he has experience, a life full of Love, Joy and Peace.
B. Diane Vchulek, MS, MA, LMHC, BCETS PTSD
Diane Vchulek specializes in helping people heal from stress reactions responses associated with both civilian and military traumatic experiences and has developed ASPIRE, a program specifically for Twelve Oaks' patients for the treatment of addiction and PTSD for military and civilians.
Diane has presented workshops nationally and internationally for International Society for Traumatic Stress Studies (ISTSS) and the International society for the Study of Trauma and Dissociation (ISSTD)
She has broad experience as a Licensed Mental Health Counselor spanning over 25 years. She was in private practice for 15 years treating individuals with PTSD and other issues related to trauma reactions. She also has clinical experience in inpatient facilities, residential treatment, and specialized programs for trauma related difficulties.
Diane's Experiences with the military throughout her life provide insight into her work with military veterans suffering from combat related traumatic stress, as well as with the family members who support them.
Diane is an Adjunct Professor in Psychology and is also currently completing her dissertation for PhD requirements focusing on “Stress Reactions in Military Wives of Deployed Spouses in Desert Storm/Shield and Operation Iraqi Freedom/Enduring Freedom”
David P. Grady, LMHC, NC
David is a Licensed Mental Health Counselor since 1993, has held a National Board Certification as a Mental Health Counselor since then and is certified as a Masters Addictions Counselor by the NBCC. He received a Master’s Degree in Counseling and Human Development in 1991 from Troy University. He also holds a Bachelor’s degree from Troy in Psychology and Sociology. His present focus is on providing treatment of adults diagnosed as chemically dependent.
In the past he has worked with domestic violence offenders and victims. He has conducted assessments and counseling for sex offenders and a variety of issues for individuals referred by county and state courts. Although he has a number of interests and specialties in the field of mental health his preference is diagnosis, treatment and aftercare planning for substance abusers.
Patty Donovan, Ph.D.
has her Ph.D in Marriage and Family Therapy from Florida State University, her Masters in Counseling Psychology from Northeastern University and her BA in Psychology and Women's Studies from Clark University. She has been a therapist for 9 years. Dr. Donovan's specializations are in sexual assault and abuse, marital counseling and chronic mental health. Dr. Donovan uses mostly brief therapy approaches when working with clients, specifically narrative therapy and solution-focused therapy.
Maria Rogado, MS
Masters in Psychology and counseling, graduate of Troy University with a background in substance dependence treatment for the past 4 years. Dedicated to the client centered approach to counseling. Demonstrating sympathetic, nurturing, non-judgmental attitude and providing and creating a comfortable environment to initiate change. Addiction is baffling, cunning, and often fatal, however I will lead you to the path of recovery, so help me help you.
Jacqueline Emond, MSCP, LMHC
has a Master of Science degree in Clinical Psychology from Troy University with the primary area in Mental Health. In Licenses and Certification she has completed all requirements for Licensed Mental Health Counselor in the State of Florida. The National Exam is scheduled. She has Teaching Certification in New Jersey and Requirements are fulfilled for Florida.
She has certification from the Florida Council against Sexual Violence Core Training, and certification in SASSI Training. She has Certification in Trauma Informed Treatment in Seeking Safety.She started with Twelve Oaks in 2008 and currently works with all Tricare patients of active duty and their dependents. She especially enjoys working with Patients with Trauma, their Families and providing the lectures. She worked in Substance Abuse and Mental Health previously handling individuals sent by State Probation and County Probation in the State of Florida.
She also managed an Adolescents in a Group home of young people removed by the state of Florida. She facilitated Reunification back into their homes. Previously she worked for the State of Florida in Protective Services, and Foster Care, and Out of Home. Jackie loves her job because she can save lives and break the cycle of Addiction and Abuse.
Annetta Davenport, MSCP
Licensed Mental Health Counselor (LMHC) and a Florida Board Certified Sex Therapist.
Training through the International Institute of Trauma and Addiction Professionals (IITAP) for certification process as a Sex Addiction Therapist.
Training through the National Institute for Trauma and Loss in Children (TLC) for certification as a Trauma Specialist.
Currently assigned to facilitate the "Chronic Pain" treatment group at Twelve Oaks.
Trained by the Department of Defense, Army, and designated as a volunteer "Master Trainer" in the Army Family Team Building program.
Cognitive-Behavioral Therapy, primarily used.
Working in the field of mental health counseling gives me a chance to be a life-long learner. I especially enjoy the area of Neuropsychology, and the specialties in addictions, trauma, and chronic pain give me the opportunity to best apply this exciting information in what I do. Because I have been affiliated with the military community as a life-long dependent in some way, I am also very sensitive to the needs of the military family.
I find that addiction (including sex addiction), trauma, chronic pain, and sex therapy are very compatible with each other, and its been rewarding to see the transformation in people when they "connect the dots" to the reasons why they may behave, and is no longer willing to use them as an excuse for unhealthy choices. I believe that the basic need humans have is to feel "safe and secure" in the relationships we have with others, and since relationship failure usually is at the core of most substance abuse, providing tools to help people develop healthy intimacy is what I truly enjoy doing. This is my way of providing hope that we all have resilience to survive, at some level, and we are capable of growth after trauma (Post Traumatic Growth).
Nicole YL Ward, MS, CMHP
Graduated from the University of Nebraska in 2002 with a Bachelor of Science in Family Science and received her Masters of Science in Human Relations through the University of Oklahoma in 2005.
Nicole has worked in the field of substance abuse and chemical dependency since 1999. She has experience with working with adolescents as well as adults in the outpatient and inpatient setting. She has worked closely with probation and other legal services to include Drug Courts, probation/community control officers, and attorneys to help clients achieve their success over addiction. She was a counselor in the United Kingdom helping adolescents become more educated about substance use and abuse as well as helping them to overcome their addictions.
Nicole grew up in a military family and is presently a military wife as well and understands the struggle that substance use and abuse plays in the military family. She has worked closely with the United States Air Force and the Office of Special Investigations (OSI) as the Drug Testing Program Manager, handling drug testing for military personnel.
